While the primary avenue of care at Park Church is through Gospel Communities, there are times when additional support is needed. If you are not currently in a group—or if you feel you need care beyond what your group can provide—you may request care by completing the confidential Care Request form below. After your form is submitted, we will be in touch to help direct you to the most appropriate avenue of care.
